What is the assessment stage?
The systematic collection of relevant data about the client for the purpose of identifying nursing diagnoses and planning nursing care
What does the nurse do in the assessment stage?
-nurse obtains facts and pieces of information
-organises and sorts information
-documents information in the form of a database
What is the nursing diagnosis stage?
Proposed statement about the clients health care need or problem that describe a healthy, unhealthy or potentially unhealthy response to illness, injury or environmental situation
What does the nursing diagnosis include?
-possible causes or factors related to the identified need and is derived through clinical decision making based on the analysis of data collected during assessment
What is the planning stage?
The development of a nursing care plan that will direct how the clients care will evolve and how the nursing diagnosis will be resolved
What is established during the planning stage?
Priorities are established along with goals, objectives and outcome criteria
Measures the effectiveness of the actions
What factors must be considered in the planning stage?
-include prioritising actual and potential problems
-sets nursing goals
-the nursing model
-appropriate interventions to resolve and alleviate problems
What is the implementation stage?
The action phase during which the nurse carries out activities and treatments aimed at resolving the nursing diagnosis
How is implantation of interventions initiated?
These are initiated according to the plan of care and may include self-care actions carried out by client and actions carried out by others
What is important to do in the implementation stage?
Documentation of actions
What is the evaluation stage?
An ongoing and intentional activity for the purpose of determining how well the client is progressing toward meeting the care goals and resolving stated nursing diagnosis
What does the evaluation stage involve?
Involves judgment of the part of the nurse as they compare the outcome criteria statement to what is actually happening to the client
What must be considered at the evaluation stage?
If the goals have not been met or have only partially been met
May be necessary to reevaluate by reentering the assessment phase and go through the steps of the process again
Examples of nursing diagnosis?
Needs support to drink adequate amounts due to polyuria and polydipsia which could lead to further dehydration or soiling in the kennel
Needs support to eat adequate amounts and correct nutrition to support lethargy
Examples of nursing goals?
To ensure the patient is meeting daily fluid requirements (50ml/kg/day) to prevent further dehydration
To increase patients food intake to meet daily RER to increase energy levels
