What is the questions to ask parents when suspecting nursing bottle caries?
Does the child take a bottle to bed?
What is in the feeding bottle?
What is the advice to provide parents with?
Replace feeder cup bottle from 6 months
No feeding at night - as there is lactose in milk, decreased salivary flow and held in mouth
Advise no on demand feeding
Wean child from bottle
No sweetened milk
Milk and water only between mealtimes
Sugarfree variations of drinks/foods/medicine
Safe snacks are- cheese, breadsticks, plain crisps
What is nursing bottle caries?
Form of tooth decay that is caused by children sleeping with bottles that contain anything other than water.
Typically affects upper anteriors and molars but lower incisors as these are protected by the tongue.
Commonly affects children between 1-2
Give tooth brushing advice to parent.
Should be started as soon as first tooth erupts
Should be assisted til age 7
Work in a systematic manner from back to front
Use a fluoridated tooth paste
Brush 2x daily for 2 minutes in morning and night
No food or drink after brushing teeth at night
Wait 30 mins in the morning after brushing to eat or drink
Spit don’t rinse
What strength and size toothpaste should be used for under 3’s?
1000ppm
A pea sized amount
What strength and size toothpaste should be used for ages 3-9?
1450ppm
A smear sized amount
What age can you prescribe higher ppm fluoride?
above age 10- 2800ppm
Provide diet advice you would give to parents.
Aim to have less than 4 sugary intakes per day
Restrict sugary drinks to meal times
Do not give feeding bottles at night
Drink only water/milk between meals
Be aware of hidden sugars
Safe snacks- veg, breadsticks, cheese
What is the treatment for nursing bottle caries?
Consider XLA of primary teeth causing pain
Potential restoring teeth that are not
Consider SDF
If pt is unco-operative refer to GA for XLA
After what age can children start using mouthwash? Strength?
