What order would you administer meds?
Oral meds (Assessment, tablets/capsules, liquids)
Transdermal patch
Eye drops
Ear drops
How would you administer Oral meds?
- Bring MAR to patient’s bedside and identify patient
- Place patient in high fowler’s
- Assess ability to swallow, determine which liquid patient uses to swallow meds
- (Know which meds cannot be crushed ex: enteric-coated, sustained-release, sublingual and buccal)
- For diabetic patients, use sugar-free food for crushed meds - Don gloves
-Administer meds (assessment, tablets/capsules/liquids)
-Hand hygiene - Document administration
-If a medication was held or refused place your initials in the dox and circle them. Document and notify the prescriber
How to administer transdermal patch?
-Don gloves
- Locate old patch and remove
- Fold patch with medication sides together
- Dispose of patch, per policy
- Controlled substance patches (ex: fentanyl) are disposed of in sharps container
- Second nurse must witness disposal, both must sign
- Inspect area for irritation/redness
- Use different sides, select for dry, intact and free of hair (CANNOT shave hair)
- Do not place hormone patches near breast
- Clean site
- Remove gloves + perform hand hygiene
- Do third check of med to MAR
- Don gloves
- Open new patch, write: date, time, initials
- Place patch, hold for 30 seconds
- Remove gloves + perform hand hygiene
- Document administration

How to administer eye drops?
- Position patient in supine or Fowler’s
- Don gloves
- Inspect eyes for contacts, drainage or crusting
- clean area from inner to outer canthus if req. - Remove gloves, hand hygiene and new gloves
- Perform third med check against MAR
- Inform patient vision may be blurry
- Ask patient to look up
- Pull down lower eyelid, expose conjunctiva
- Administer drops into conjunctiva
- If drops miss, wipe area and administer again - Ask patient to close eyes
- If drops have systemic effects, apply pressure to nasolacrimal duct for 1 min
- Remove gloves and perform hand hygiene
- Document administration

How to administer ear drops?
- Warm drops between hands; cold drops may cause pain,dizziness
- Position patient on side
- Don gloves
-Inspect ear for cerumen/drainage; clean if necessary
- Remove gloves, perform hand hygiene and don new gloves, if cleaning was conducted - Perform third med check against MAR
- Pull ears up and back for children and adults over 3yo
- Pull ear down and back for kids 3 or younger
-Instill drops- Massage tragus to help medication move
- Remove gloves and perform hand hygiene
- Have patient remain on side for 10 minutes
- Assure patients safety
- Document administer meds
