Q

What is an SMS?

A

Safety Management System, is a systematic approach to managing safety.

Your vessel’s SMS should be based on a risk assessment of your operations. It should describe how safety, maintenance and operation is managed on your vessel

The owner, operator, master, and crew of each vessel should all be involved in carrying out the risk assessment and in developing, reviewing and updating the safety management system.

Q

What are SDS’s?

A

Safety Data Sheets are;
documents that list information relating to occupational safety and health for the use of various substances and products. SDSs are a widely used system for cataloging information on chemicals, chemical compounds, and chemical mixtures
Must be less then 5 years old

1. Limited openings for entry and exit
2. Inadequate ventilation
3. Is not designed for continuous worker occupancy

Safe Work Australia,
An enclosed or partially enclosed space that;
1. Is not designed or intended to be occupied by a
person
2. Is, or is designed or intended to be, at normal
atmospheric pressure while any person is in the
space
3. Is or is likely to be a risk to health and safety from;
. An atmosphere that dose not have a safe oxygen
level
. Contaminants, including airborne gases, vapours
and dust, that cause injury from fire or explosion
. Harmful concentrations of any airborne
contaminants

                 substances
Annex 3:
17
Q

Explain a work plan for a job?

A
1. Consult
   . Plan the job
   . Risk asses the job
   . Consult the OEM manuals
   . Consult an expert
2. Isolate the equipment
. Isolate as close to the source as possible
3. Lock out tag out
4. Confirm isolations
. Test and retest (Test for Dead)
   . test multimeter before use
5. Retest isolations
6. Document
   . Review the JHA
   . Review the SOP
18
Q

Can vessel’s discharge bilges overboard?

A

Yes as long as the vessel is proceeding on route and oil concentrations are no greater then 15 parts per million (ppm) and not with in port limits

19
Q

Name the relevant MARPOL Annexes

A
Annex 1: oil
Annex 2: Noxious liquid substances
Annex 3: harmful packaged substances
Annex 4: sewage from ships
Annex 5: Garbage from ships
Annex 6: Air pollution
20
Q

What is the requirements of a vessel to comply with MARPOL

A

The vessel must have an International Oil Pollution Prevention Certificate (IOPP)
The vessel must use an oil record book for engine room
And one for cargo operations

21
Q

What are some of the main elements in an SMS

A

. Vessel contact details
. Risk assessments of vessel operations
. Owners responsibility and authority statement
. Designated person (DP)
. Masters Responsibility and authority statement
. Minimum and appropriate crewing identified
. Operation procedures established
. Emergency procedures established
. Hazardous occurrences investigations
. PMS, Planned maintenance system
. Vessel documentation, ie log book, crew lists ect
. SMS reviewed annually

22
Q

What is a Hazard?

A

A hazard has the potential to cause death, injury, illness or environmental damage, for example: 1. the environment in which the vessel operates 2. unsuitable vessel 3. machinery plant and lifting devices
4. inadequate safety systems
It’s everyone’s responsibility to identify hazards.

23
Q

What is a risk?

A

Risks arise from interactions with one or more hazards that may have detrimental impact on safety.

24
Q

What is a risk assessment?

A

The process of evaluating the likelihood of an incident or injury
. What could cause harm (hazard)
. What could go wrong (risk)
. How likely is it to happen (likelihood)
. How bad will it be (consequences)

25
Q

What are some risk controls?

A
. Elimination
. Substitution
. Isolation/engineering
. Administration
. PPE, Personal Protective Equipment
26
Q

What dose the SOLAS fire divisions refer to

A

A Class division
. Made from steel or equivalent material
. Can prevent the passage of smoke and flame, 1 hour
. Must prevent heat transfer to the unexposed side
Rated as A60 ie will not reach 139deg in 60 min
