Alert and cooperative patient
A pathology that can be reversed acutely (e.g. CHF, COPD, etc…)

A
Severe agitation Copious secretions Inability to protect airway Repeating hemoptysis Facial trauma Recent esophagectomy

Contraindications of NVVP (relative)
A
Mild Decrease in LOC Progressive severe resp failure Uncooperative pt that cannot be calmed Suspected acute cardiac syndrome Hemodynamic instability Pregnancy

Initial Settings of NPPV
A
10/5 (Not < 8/4)
Vt 5-7ml/kg ideal body weight
SpO2 > 90%
Increase IPAP by 2cmH2O if persistant hypercapnia
Increase IPAP and EPAP by 2cmH2O if persistant hypoxemia
Max IPAP 20-25cmH2O to avoid gastric distension
