Samter’s triad:
Aspirin allergy, nasal polyps, asthma
Epistaxis tx
Direct pressure 5-30 min and if unsuccessful, Rq silver nitrate cauterization
- petroleum jelly or bacitracin applied 3-4x/d to the nares w. Qtip
- ant/post nasal packing if cauterization is unsuccessful; removed 3-4d later; empiric abx; ENT consult
Dx procedure of choice for a foreign body
Physical exam
Oxymetazoline HCL
(Afrin) used as topical vasoconstrictor in “draining the sinuses” tx for Rhinosinusitis
- limit to 3-5 days bc can cause rhinosinusitis medicamentosa
Phenylephrine and pseudoephedrine
Tx as oral vasoconstrictors in draining the sinuses w/ rhinosinusitis
Mucolytics
I.e. Guanefesin, robitussin, mucinex–> help drain the sinus in rhinosinusitis
Intranasal steroids
Use 15-21 days; help drain the sinus in tx for rhinosinusitis
Saline lavage
help drain the sinus in tx for rhinosinusitis
- may be helpful for those w/ CV probs or on multiple meds
These meds should not be used in tx for rhinosinusitis
Antihistamines 1st generation (increase mucous viscosity)
Acute sinusitis tx for community infx
Amoxilicin- clauvanate acid (augmentin)
- 2 g BID PO
Or
- 90 mg/kg/day PO
Nosocomial sinusitis pathogens
Almost always polymicrobial: Klebsiella, pseudomonas, enterobacter, proteus, and E. coli
Sinusitis is MC in what time and gender
Early fall to early spring; women
Sinusitis w/ anaerobes?
Following dental dz or chronic sinusitis
Normal flora sinusitis
H flu, m catarrhalis, s. Pneumoniae, and s. Aureus
