Non-pharmacological treatment of arrhythmia Flashcards
1
Q
List the different non-pharmacological treatments that can be used to treat arrhythmias:
A
- catheter ablation
- ICD and pacemaker
- CRT
- electrical cardioversion
- Defibrillator
2
Q
Catheter ablation: def
A
- removal of tissue by electrical energy through a catheter
3
Q
Catheter ablation: radiofrequency energy
A
- cells are destroyed by heating > 50 degrees
4
Q
Catheter ablation: cryoablation
A
- liquid nitrous oxide is released into the tip, rapidly vaporizes and removes heat from the tissue in contact with the catheter, temperature falls to -30 -> reversible loss of cell function -> cooled to -60 irreversible
5
Q
Catheter ablation: indications
A
- AVNRT (97% success rate)
- AVRT
- A-flutter
- A-tachy
- PVC
- A-fib
6
Q
Catheter ablation: complications
A
- death, stroke, tamponade, AV node block, coronary spasm, x-ray exposure, pericarditis, recurrence of arrhythmia, cardiac perforation
7
Q
Catheter ablation: advantages
A
- reduces ICD shocks, high success, well defined lesion, no adverse hemodynamic effect, short recovery, repeatable, cost effective
8
Q
Ablation strategy of ventricular arrhythmias (VT)
A
- Exit - point of ablation -> de-channelization -> core ablation -> LP and LAVA ablation -> scar homogenization
9
Q
CRT: Cardiac resynchronization therapy:
A
Biventricular pacing. Device paces both the left and right ventricles simultaneously, improves EF. Can restore function: ex: in dilative cardiomyopathy, improves CO and symptoms
10
Q
Electrical cardioversion
A
- indications: sustained VT, treatment resistant SVT, A-fib, AFL
