A

When an exhibit is transferred between individuals there needs to be a clear record of who is responsible for the item and all associated documentation needs to be completed.
Documetation proves whether a chain of custody was correctly maintained.

A

Name of the person collecting the item.
The exact identity of the item is.
Exact location of where is was found.
Date/time of when it was collected.
