Indication
In patient where gastric distention is probably or existing
Indications 2
All pediatric intubated patients
Contraindication
Nasogastric insertion is contraindicated with patients having suspected head or facial trauma
Procedure 1
Standard Precautions
Procedure 2
Assemble and test all necessary equipment (tube, tape, syringe, flush, suction)
Procedure 3
Note: Nasogastric= Conscious, Orogastric = Unconscious
Procedure 4
If conscious, explain the procedure and remind them not to move
Procedure 5
Prepare the patients head into a neutral position (unless contraindicated) while preoxygenation
Procedure 6
Determine and mark the length of tube insertion by measuring from:
Oro= from mouth to ear to xyphoid process
Naso= from nose to ear to xyphoid process
Procedure 7
If the patient is awake use topical anesthetic to the nares or oropharynx
Procedure 8
Lubricate the distal tip of the gastric tube and insert into the nare along the nasal floor, alternatively into the oro cavity at midline
Procedure 9
Advance the tube gently. If the patient is awake, encourage swallowing to facilitate the tube’s passage. Advance to the predetermined mark on the tube.
Procedure 10
Confirm proper placement by injecting 10-30ml of air with syringe and auscultating the epigastric region for air sounds. Inability to speak properly indicates incorrect position of the tube in the vocal cords and into trachea and must be removed.
Procedure 11
If desired , apply suction at the LOWEST setting, note the contents that pass through the tube.
