dysphagia, dysarthria, dysphonia,
nystagmus, vertigo –> falling
loss of pain, Horner’s syndrome (ptosis, decreased sweating, pupil constriction) on one side of the face
sensory loss on the other side of the body.
Dx? which side?
PICA infarct (Posterior inferior cerebellar artery)
=lateral medulary syndrome
=Wallenberg syndrome
facial side
partial seizure without losing consciousness.
Dx and Tx?
Dx : simple partial seizure (Focal Onset Aware Seizures)
Tx : carbamazepine
chorea two months after streptococcal throat infection.
Tx?

sudden stares, fidgeting of hands, fingers, lip smacking, chewing movements, 3-4 episodes per day, each lasting 60-90 seconds, post-ictal confusion.
Dx? Tx?
Dx. temporal lobe epilepsy

Tx. carbamazepine
Guillain-Barre syndrome Tx?
IVIG
unexplained falls, dysphagia, axial rigidity, vertical gaze deficits
supranuclear palsy
Hx of strep throat,
uncoordinated movements affecting hands, feet, tongue and face.
Dx and Tx?
Sydenham chorea due to rheumatic heart disease.
Tx : carbamazepine or valproate
diabetic neuropathy.
what to test before starting pregabalin?
- VitB12 (metformin can cause low Vit B12)
- TFT to rule out hypothyroidism
- autoimmune neuropathy
parkinson’s disease, 0nly resting tremor.
Tx?
L-dopa (may not work)
clozapine (may be helpful but weekly check on WBC count)
benzhexol or benztropine (anticholinergics) : often used but with side effects.
Sx of pure motor stroke.
unilateral lower facial, limb weakness
hyperrelexia on the same side limbs
no sensory deficit.
The most likely lesion of the brain?
internal capsule!
It’s a lacunar infarction.
Tx for trigeminal neuraligia?
carbamazepine
Tourette syndrome Tx
Haloperidol (Haldol)
pimozide (Orap)
aripiprazole (Abilify)
tonic clonic seizure, the first line treatment?
valproic acid
