epilepsy dx
2 or more unprovoked seizures
single unprovoked seizure
no discernable acute severe systemic metabolic fx alteraion or acute insult to structure of brain
no dx of epilepsy but @ higher r/f subsequent episodes
WP = 5 years of SFT
max cert = 1 year
acute seizure d/t metabolic illness
no known associated w/ subsequent episodes
Max cert 2 years if:
–metabolic dysfx is corrected
–no DQing r/o recurrent of seizure causing condition
seizure exemption
very rarely given
case-by-case
contingent on driver maintaining stable tx regimen and SF during 2 year period
requires annual report from neurologist of SFT
Severe TBI
penetration of dura and LOC > 24h
high r/o unprovoked seizure which does not diminish overtime
DQ (w/ or w/o early seizure)
Moderate TBI
no dural penetration; LOC > 30 min but < 24h
w/o early seizure –> max cert = 1 year
w/ early seizure –> WP = 5 years seizure free and off meds
Mild TBI
no dural penetration; LOC < 30 min
w/o early seizure –> max cert = 2 years
w/ early seizure –> WP = 2 years seizure free and off meds
TBI certs depend on
completed WP
normal PE
normal neuro exam
testing from and neuro-ophth & neuropsych
neurology clearance
TIA
cerebral blood supply is decreased for 20 minutes w/ resolution of sx and no lasting neuro deficits
WP = 1 year
max cert = 1 year if
–asymptomatic
–completed WP
–cleared from neuro
–SF & not on seizure meds
–no neuro deficits
–driving unaffected
Strokes and hemorrhages
criteria = same
WP dependent on lesion location
max cert 1 year if:
–asx
–completed WP
–cleared from neuro
–SF & not on seizure meds
–no neuro deficits
–driving unaffected
cerebellum or brainstem stroke/hemorrhage
low seizure risk
WP = 1 year
cortical or subcortical stroke/hemorrhage
high seizure risk
WP = 5 years

DQ if anticoags are part of medical tx –> high r/o bleeding complications
autonomic neuropathy
nerves innervating vital fxs (heart and smooth muscle) are affected
certify if nature and severity does not endanger public safety

inclusion body myositis
polymyositis
metabolic muscles dz

Bacterial meningitis
w/o early seizure –> WP = 1 year
w/ early seizure –> WP = 2 years
Viral encephalitis
w/o early seizure –> WP = 1 year
w/ early seizure –> WP = 10 years
