Max cert = 1 year if obtained ITDM assess from treating physician (must show stable regimen and control)
must receive assessment no later than 45 days from signed form
driver submits 3 months of self-monitoring BGC levels while on insulin.
–if unable to provide log, cert x3 months so they can supply them
DM DQs
severe hypoglycemic episode (see card)
orthostatic hypotension
peripheral neuropathy
loss of position sensation
Non-proliferative and proliferative diabetic retinopathy
