A

evaluates the consciousness level of a patient. allows time for interventions to be performed

A

conscious level from the patient’s verbal, motor and eye opening responses.

how do you measure the GCS score eye opening response?

A

4- the best eye response, spontaneous and unprompted.
1- minimum score indicates no eye opening to pain.
however if there is no response due to primary ocular cause (e.g swelling), then C for closed should be documented.
