What does AVPUC mean?
A - alert
V - voice
P - pain
U - unresponsive
C - confusion (new confusion)
what is included in a neuro assessment?
- GCS
- pupil response
- limb power
- vital observations
What is the GCS?
glasgow coma scale
what is the GCS used for?
evaluates the consciousness level of a patient. allows time for interventions to be performed
how is the GCS scored?
scored out of 15, 3 as the lowest
What is the normal GCS score?
9 - 12
What does the Glasgow Coma Scale assess?
conscious level from the patient’s verbal, motor and eye opening responses.
how do you measure the GCS score eye opening response?
4- the best eye response, spontaneous and unprompted.
1- minimum score indicates no eye opening to pain.
however if there is no response due to primary ocular cause (e.g swelling), then C for closed should be documented.
how do you measure the GCS verbal response?
5- best verbal response, the patient is being oriented to time, place and person.
1- there is no verbal response.
how do you measure the GCS motor response?
6- best motor response, the patient can obey commands and instructions.
1- no motor response.
