Neuro - Focused Hx Q
-hx of neurological problems, any head trauma, loss of consciousness, dizziness, head aches or seizures.
-Any memory problems, forgetfulness, inability to concentrate?
-Any changes in sense of spell, taste, hearing, feeling or balance?
-Any weakness, numbness or paralysis?
- Difficulty with ADLS?
- tremors or involuntary movements?
- mood problems or depression?
Alcohol or drug use?
-ever been treated for neurological or psychiatric problmes
-Hx of hypertension, diabetes, stroke or circulation?
Brief Neuro Exam( 4)
- level of conciousness
- glasgow coma scales
- pupillary response
- Vitals
Unconcious
still aroused by painful stimuli
unresponsive
not aroused by verbal, tactile or painful stimuli
Coma
Prolonged unconsciousness, unresponsiveness
Glasgow coma scales ( three assessments and their highest score)
- eye ( highest score 4)
- motor ( highest score 6)
- verbal ( highest score 5)
Results of Glasgow coma scale (6 states)
- Mild ( 13-15)
- Moderate ( 9-12)
- severe disability ( 3-8)
- Vegetative state ( < 3)
- Persistent vegetative state (lasting longer than one month)
- Brain death
Decorticate
abnormal bodily position in which hands are curled in towards chin
Decerebrate
abnormal bodily position in which hands at side curled in thumbs down.
Abnormal pupills
Inequality of > 1mm
Dolls Eye movement (oculocephalic Reflex)
Abnormal: Eyes movements follow the head movement
Normal: Eyes move opposite of head- aka they can stay fixed on an image
Cerebellar function tests
observe gait ( smooth, rhythmic, effortless, opposing arm swing tandem walking ( heel to toe)
Romberg Test
Close eyes, with feet together
Abnormal: excessive swaying/ loss of balance
Normal: mild sway, can hold with eyes closed for 20 seconds
Coordination and skilled movement tests (4)
- finger to nose, with eyes open and then closed
- finger( mine) nose ( theirs) finger ( mine)
- Heel to shin
- finger to finger
Where do you test Reflexes (5)
- tricep
- bicep
- Brachioradialis
- Pateller
- Achilles
