NBME Flashcards
What is tonometry?
test used to measure pressure in the eye; used to screen for glaucoma
Simple Partial Seizures
begin in focal area of the brain and do not impair awareness; generally associated with positive phenomena (tingling, hallucinations)
Complex Partial Seizures
focal onset and involve impairment in awareness; commonly arise in temporal lobe but can involve frontal lobe as well; may include automatisms +/- speech arrest
Steps to lower ICP
- Elevate head of bed
- Hyperventilate
- Mannitol
Stroke Precautions: embolic
- Permissive HTn
- Head of bed flat
- IV normal saline
- Maintain normoglycemia, normothermia
Stroke Precautions: hemorrhagic
- Lower Bp
- Head of bed to 30 degrees
- Correct fevers, glucose abnormalities
- Reverse anticoagulation
Pathophys: organophosphate poisoning
inhibition of AChE, leading to accumulation of ACh in synaptic cleft
Organophosphate poisoning: signs and sx
SLUDGEM (Salivation, Lacrimation, Urination, Defecation, Gastrointestinal motility, Emesis, miosis)
Pathophysiology of heat stroke
thermoregulatory center fails to dissipate heat at rate necessary to maintain euthermic state
MS: CSF analysis
Oligoclonal bands in the presence of normal protein concentration
predominant immunoglobulin type in MS CSF?
IgG (although IgM, IgA also increased)
Vessels responsible for lacunar strokes
In order of decreasing frequency: Basal ganglia (esp putamen) Thalamus Pons Cerebellum Cortex
Med used for young pt with Parkinson’s who present with tremor…
trihexylphenidyl (anticholinergic)
Pathology of hyponatremia in subarachnoid hemorrhage
- Inappropriate vasopressin secretion (water retention)
2. Increased ANP/BNP secretion (salt wasting)
