2 main pathogenic species
Mycoplasma pneumoniae and Ureaplasma urealyticum
Mycoplasma
tiniest exists on outside of cell, draws nutrients (like cholesterol) from drawing them out from cell it attaches to. Mycoplasma attaches to ciliated and non-ciliated cells. has no cell wall but has a three layered sterol membrane
Mycoplasma in culture
extremely slow growing, very fastidious , produces a fried egg colonies, however, M.pneumoniae do not produce these colonies.
Culture has been discontinued, now use molecular assay
M. pneumoniae
Causes an upper respiratory infection and pneumonia, transmitted via respiratory droplets produced by coughing. infection of the respiratory tract has a 2 to 3 week incubation period, onset looks like flu, with a dry non productive cough
M.hominis
very aysmptomatic, loose association but will present as PID, more prevalent in sexually active females
Dermatologic conditions associated with M.pneumoniae
can occur in 7% of people, a sever skin reaction of erythematous vesicles and bullae over mouth, eyes and ski, more common in men - Steven Johnson syndrome
Also can have Raynaud’s phenomenon- vasospasms of fingers when exposed to cold, more common in women- whitening of fingers, and spasmodic pain.
diagnostic features of M.pneumoniae
cold agglutins test and complement fixation test, IgM assay
ureaplasma urealyticum
causes urethritis, not typically….
