What is myasthenia gravis?
Autoimmune disorder of the neuromuscular junction
In myasthenia gravis, what are the antibodies directed against?
postsynaptic acetylcholine receptors
What are the symptoms?
Fatiguability - symptoms worse throughout day
Weakness - extra ocular, bulbar, facial weakness
What are some eye symptoms?
ptosis
diplopia
What can worsen the weakness is myasthenia gravis?
pregnancy hypokalaemia infection emotion exercise change of climate
What is the cause of myasthenia gravis?
antibodies to nicotinic acetylcholine receptors –> reduced number of functioning receptors –> muscle weakness and fatiguability
How is myasthenia gravis diagnosed?
anti AChR antibodies (90%)
What is it associated with?
thymus hyperplasia or thymoma
other autoimmune diseases
What is the treatment of myasthenia gravis?
anti cholinesterase e.g. pyridostigmine
immunosuppression e.g. prednisolone, azathioprine, methotrexate
thymectomy
What is the main complication of myasthenia gravis?
myasthenic crisis - weakness of resp muscles during a relapse
