3. Verify person making request is authorized to do so.
4. ID/History of persons involved, if possible
5. ID/History of location involved, if possible
6. Threat assessment (SWAT, etc)
7. Identify location, personnel and equipment needs
8. Notify CoC
9. Notify CID, if applicable
10. Complete Mutual Aid worksheet
11. Forward worksheet to Major
12. Significant Event Log
