Multisystem Flashcards
What is the goal of treatment of fluid resuscitation in a patient with hypovolemic shock?
The goal is to maintain O2 delivery (DO2) and O2 uptake (VO2) into tissues and sustain aerobic metabolism
What is the endpoint of volume resuscitation for oxygen consumption?
250ml/min
What is a normal SvO2?
60-75%
What will your SvO2, SVR and PAOP look like in a patient with sepsis?
SvO2 will be high (>75%) due to impaired cellular oxygenation saturation. Cells can’t extract O2 from the blood or the tissues beds are not well perfumed with oxygenated blood.
PAOP and SVR will be low as vasodilation leads to decrease venous return and decrease afterload
Which of the following is TRUE in regard to shock?
A. The MAP is adequate in the compensatory phase of shock
B. The blood pressure is maintained in Class III hemorrhagic shock
C. An elevated lactate level occurs late in septic shock
D. A serum bicarbonate is elevated in shock
*A.
Since the compensatory mechanism are working the MAP is maintained in the compensatory phase. If these mechanisms fail the MAP drops and hypotension results which would be during the progressive phase of shock.
The blood pressure is maintained in Class II
Septic shock lactate rises early during sepsis
serum bicarb is decreased as the patient’s lactate level is rising
A patient is being treated for a confirmed salicylate overdose. Which of the following interventions should the nurse anticipate?
A.closely monitor for respiratory depression
B. prepare for hemodialysis
C. administer thiamine 100mg IV
D. administer Mucomyst
B
Salicylate toxicity will require dialysis in order to prevent acute renal failure, even if the renal labs are normal.
What is HELLP syndrome and who is at risk?
Hemolysis Elevated Liver enzymes levels and Low Platelet levels
it is associated with women with pre-eclampsia and eclampsia
what is the target blood glucose range for a patient with sepsis?
140-180
