Multiple sclerosis Flashcards
What is multiple sclerosis
T4 Hypersenisitivity against myelin protein of ougodendrocytes = Demyelination of CNS
What are the hallmarks of multiple sclerosis
Multiple plaques of demyelination of CNS Neurones
What are the features of MS
Optic neuritis (Cant see red) Internucleur Opthalmoplegia (Cant gaze) UMN signs Lhemitte phenomenon (shock) Uthoff's Phenomenon (Worse with heat) Charcot Neuro triad
What are the 3 classifications of MS
Relapsing-Remitting
Primary progressive
Secondary progressive
Primary Progressive
What criteria is used in MS
McDoanlds
-2< attacks disseminated in time and space
How is MS investigated
Mcdonalds criteria
MRI - GD-enhancing plaques
-Where = white demyelination
-Length = give Gadolinum contrast
Lumbar Puncture = IgG OLIGOCLONAL Bands
Clinical
How is MS managed
MDT
Acute = IV Methyprednisolone
Chronic =B Interferon
What is Uthoff’s Phenomenon
Worsening demyelinating Sx exacerbated by heat
What is Lhermite Phenomenon
Electric shock w/ neck flexion
What is Charcot’s Neuro triad
Dysarthria
Nystagmus
Intention tremor
Why is optic neuritis an important sign in MS
It is the first sign and helps differentaite between other diseases
What is Internucleur opthalmoplegia
Impaired lateral gaze
