SOCRATES RA
S - MCP, PIP, large joints O - acute C - ache A - stiffness, swelling, warmth, rheumatoid nodules, fever, dry mouth/gritty eyes T - early morning stiffness >60min E - rest S - sever (less than gout/sepsis)
4 questions about site of problem
One/more than one joint, pattern of joint involvement, small/large joints, symmetrical
Eg. acute, sub-acute and chronic onset
Acute = gout, sub-acute = RA/septic, chronic = OA
Where does neck pain radiate to?
Upper limb
Where does lower back pain radiate to?
Buttock
Where does hip pain radiate to?
Knee
3 systemic symptoms for vasculitis/connective tissue disease/IBD?
Weight loss, fever, abdo pain, fatigue, malaise, depression
3 conditions a rash may indicate
Psoriatic arthritis, SLE arthritis, IBD (erythema nodosum)
10 completing questions MSK history?
Previous episodes/treatment/response, history trauma, IBD/psoriasis/coeliac, SLE/thyroid/autoimmune, recent infection (inc dental/pharyngeal), TB, RF for gout (alcohol, low dose asprin, renal disease, metabolic syndrome (hypertention/obesity/diabestes/vascular disease)
6 social Hx questions?
Occupation (risk/impact), sports/hobbies, ADL, alcohol, smoking, home circumstances (stairs/dependants/carers/social support)
6 family history questions/conditions?
RA, OA, psoriasis, gout, IBD, CTD, autoimmune
