Transference
when a patient redirects (transfers) their feelings about a person from their past onto the nurse
Counter-transference
the nurse transferring feelings about a person from their past, onto their patient
-the inverse of transference

Tangential thinking
thoughts are connected but go off-topic and don’t return to the topic
Circumstantiality
thought process that goes off-topic but eventually makes it back to the topic
Thought blocking
when someone’s train of thought is suddenly interrupted
Confabulations
involves creating false memories or stories without intending to deceive
Neologisms
thought disorder that involves creating new words or expressions when speaking
Flight of ideas
rapidly shifting thoughts and ideas– people with flight of ideas may speak quickly and jump between ideas that are loosely connected.
Perseverations
a cognitive process that involves having difficulty stopping negative thoughts
Looseness of association
a thought process disorder that causes a person to have disorganized speech
Word salad
a thought process that involves speaking in a way that is incoherent and illogical

an unshakable belief in something that’s untrue– a THOUGHT CONTENT
Types of perceptions

Illusion
a false perception of a real thing– a PERCEPTION
-ex. thinking a pillow is a dog
Depersonalization
feeling like you are not yourself or a part of you doesn’t belong to you– a PERCEPTION
Derealization
feeling like you are looking at everything from a distance/ like it is a movie– a PERCEPTION
What should you state for judgment and insight?
intact or not intact
Judgment
client’s ability to problem solve and choose among alternatives based on reality
Insight
the client’s self knowledge about his current illness and problems
Types of cognitive functioning
-orientation (person, place, time)
-memory (remote, recent)
-abstraction (concrete, abstract)
