Diagnostic criteria of RRMS
- demylinating CNS which are: disseminated in space (MRI/clinical) OR dissemintaed in time (MRI/cliical)
define:
- relapsing remitting
2. progressive MS
- symptoms occur and then resolve
2. lesions occur but do not get resolved
work-up for MS patient
- MRI
- CBC, CMP
- B12, TSH, ESR, CRP
- ANA, RF, anticardiolipin
- LP (oligoclonal bands, IgG and cell count)
- visual evoked potential
- measure retinal nerve fiber layer thickness (optic coherence tomography)
acute therapy versus chronic therapy of MS
acute: steroids, plasmapharesis
chronic: glatamir acetate, interferon, dimethyl fumarate, teriflunomide, fingolimod, natalizumab, alemtuzumab, ocrelizumab
MS mimicker:
neuromyelitis optica- optic neuritis (both eyes)
which HLA with MS
DRB1
risk factors for mS
vit D deficiency, female, norther latitiude, mononucleiosis, smoking, obesity
uhthoff phenomenoso
transient workening of symptoms with temperature
lhermitte sign
paresthesia radiating down trunk when neck is f;exed
diagnostic criteria of progressive MS
> 1 y progression

name the side effect:
- natalizumab
- fingolimid
- dimethyl fumarate
- ocrelizumab
- PML
- bradycardia
- decreased leucosite
- contraindicated in hep B +
neuromyelitis optica
aquaporin4 IgG
can have transverese myelitis
ADEM- acute disseminated encephalomyelitis
follows immunization or infection
- demylinating of central nervous system
- acute onset and rapid progression
- multifocal neurolic deficits
ADEM TREATMENT
steroids and acyclovir
