opioid analgesic

What are the CNS effects of morphine
A
- CNS depression (leading to analgesia)
- Respiratory depression
- Depression of cough reflex
- Stimulation - euphoria, nausea, vomiting, pin point pupils, dysphoria
- Dependence

What are the CV effects of morphine
A
- Vasodilation

What are the primary emergency indications for morphine
A
- Pain relief
- LVF w/ SOB and full field crackles
- Sedation to enable and maintain intubation
- RSI

What are the primary contraindications for morphine
A
- History of renal failure/impairment
- Hypersensitivity
- Late 2nd stage labour

What are the primary precautions for morphine
A
- Elderly/frail
- Hypotension
- Respiratory depression
- Current asthma
- Respiratory tract burns
- Known addiction
- Acute alcoholism
- Monoamine oxidase inhibitors

Onset, peak and duration of morphine
A
IV:
Onset = 2-5 minutes
Peak = 10 minutes
Duration = 1-2 hours
IM:
Onset = 10 - 30 mins
Peak = 30-60 mins
duration 1- 2 hours

CNS:
- Drowsiness
- Respiratory depression
- Euphoria
- Nausea, vomiting
- Addiction
- Pin point pupils
CV:
- Hypotension
- Bradycardia
