Blocks pre-synaptic a2 receptors (autoreceptors and heteroceptors) preventing self mediated regulation fo NE and 5HT levels by the neurons and increasing synaptic cleft NE and 5HT:
Mirtazapine (Remeron)
Used to treat bipolar disorder
no effect on normal subjects
unknown MOA, likely post synaptic neuron involvement
readily absorbed from GI tract, 95% renal excretion
LOW therapeutic index
Lithium Carbonate (Eskalith)
Anticonvulsant used to treat bipolar disorder
better than Lithium for rapid cycling bipolar disorder
appropriate first line tx for some
can be combined with Lithium for pts who have failed either treatment alone
Valproic acid (Depakote Depakene)
Anticonvulsant approved by FDA for prophylaxis of bipolar disorder
Carbamezepine (Tegretol)
