Core symptoms of depression
Low mood, loss of pleasure (anhedonia) and loss of energy (anergia)
Diagnosing depression
Sx must be present on most days for two weeks

2+ episodes of depression

Self help, lifestyle adjustments
Treatment for moderate depressive episode
CBT +/- sertraline
treatment for severe depressive episode
Intensive meds, secondary care teams
Treatment for psychosis
Olanzapine
Hypomania
Elevated or irritable mood for 4+ days
Symptoms of hypomania
- Increased actviity
- Restlessness
- Increased talking
- Increased sociability
- Decreased sleep
- Poor concentration
- Mildly reckless behaviour
- Increased sexual energy
It has modest functional impairment
Symptoms of mania
3+ of the following:
- Increased activity
- Increased talking
- Flight of ideas
- Loss of normal social inhibitions
- Decreased need for sleep
- Increased self esteem
- Poor concentration
- Reckless behaviour
- Marked sexual energy
It has severe functional impairment
Bipolar disorder
- One episode of elevated mood and one episode of other affective disorder (depression, mania, hypomania or mixed affective episode)
- Some cycles are predictable but others aren’t
- Manic episode followed by depression
Treating bipolar
Self-care, CBT, mood stabiliser (lithium or olanzapine)
