Bloods, normal and cultures,
Aspirate (septic!) plus microscopy and polarised crystals

What’s the treatment for acute gout?
A
NSAIDs
Colchicine
Steriods
Chronic - allopurinol

What is seen in gout aspiration?
A
Gout - negatively bifringent crystal needles (urate )
Pseudogout - positively bifringent rhomboids. (Calcium pyrophosphate)
