IgA deposition in the small vessels of the superficial dermis - detected via skin biopsy
Clinical features of nephrotic syndrome
Proteinuria
Hypoalbuminuria
Oedema
high c3
Clinical features of nephritic syndrome
haematuria
puffy eyes or swollen ankles
oligouria
low C3 that normalises in 3-4 weeks
Features of Henoch-Schönlein Purpura?
skin rash on buttocks and extensor surface of arms and legs
arthralgia/arthritis
periarticular oedema
abdominal pain
glomerulonephritis
Complications of nephrotic syndrome
thrombosis - hypercoagulable state (lose protein C and S)
cellulitis
hypertension
hypovolemia - loss of intravascular fluid
bleeding
Treatment of nephrotic syndrome
Steroid daily for ~4 weeks until proteinuria is 0 or 1+ then wean down the amount of steroid via alternating days (over a few weeks).
Consider albumin and frusemide
