Maintenance calculation
100 + 50 + 20 rule / 24hr
4 + 2 + 1 rule
Replacement calculation
Maintenance + deficit - bolus
e.g. 25kg child 5% dehydrated
Maintenance = 1600ml (1000 + 500 + 100)
Deficit = 25 kg x 5% = 1250
Replacement = 2850/24 = 118ml/hr
Fluid types used in maintenance, replacement, and resuscitation
Maintenance = 0.9 NaCl, 5% glucose, +/- KCl (20mMol)
Replacement = 0.9 NaCl, 5% glucose, +/- KCL (20mMol)
Resusc = 0.9 NaCl
Resusc fluid amount
10ml/kg bolus of normal saline
Fluid given in mild dehydration (i.e. trial of fluid)
0.5ml/kg every 5mins. Try oral/enteral first
Symptoms of mild, moderate, and severe dehydration in regard to:
HR
RR
skin turgor
frontonelle
cap refil
peripheral temp
mood/conscious state
skin colour
mucous membranes
pulse
