Module 5: Unit E Flashcards
What is the difference between Suboxone and Subutex? Who is Subutex indicated for?
The drug Subutex is buprenorphine alone. It can be used as maintenance therapy or as a taper to facilitate detoxification.
Suboxone is a combination of buprenorphine and naloxone. The addition of this antagonist is to discourage IV drug use. If Suboxone is taken IV, withdrawal will occur.
What is MAT?
Medicated assisted therapy
What treatment drugs in the module prolong Qt interval?
Methadone, cocaine, buprenorphine (subutex)
Opioid antagonist example
Narcan
Indication for Narcan
Opioid overdose
MOA Narcan
Opioid antagonist
block or reverse opioids by competitively occupying the receptor site that the narcotic occupied.
Safety/D2D/etc for Narcan
Reverses the effects of respiratory depression, hypotension, and sedation.
Due to half-life, it is possible for a person who was revived to relapse back into respiratory depression after 30 minutes.
MAT Opioid example
Methadone
Indication methadone
long-term management of opioid addiction.
Safety methadone
The use of methadone in treating drug addiction is restricted to Substance Abuse Mental Health Service Administration (SAMHSA) and state-certified programs.
Prolongs QT
MAT
Opioids Partial agonist example
Buprenorphine (Subutex)
Subutex indication
Opioid use disorder
MOA subutex
Partial agonist at mu-opioid receptors and a full antagonist at kappa receptors
Subutex safety
When used in addiction treatment, do not begin the partial agonist until withdrawal from the opioid has already started.
MAT
Mixed
example
Buprenorphine and Naloxone (Suboxone)
Indication Naltrexone or Buprenorphine
Opioid use disorder is used to discourage the return to the use of opioids after detoxification.
Used in the treatment of alcohol abuse and off-label for weight loss.
Safety opioid antagonist Naltrexone
Naltrexone is contraindicated in patients who are active narcotic users, have severe liver disease, and/or have moderate to severe renal disease.
BBW Naltrexone
hepatocellular injury may occur with excessive doses
Medication for Maintenance of alcohol abstinence
Acamprosate calcium (Campral)
Indication for Acamprosate calcium (campral)
maintenance of abstinence from alcohol by reducing alcohol cravings
MOA Acamprosate calcium (Campral)
possibly by blocking glutaminergic N-methyl-D-aspartate receptors, while gamma-aminobutyric acid type A receptors are activated.
Safety Campral
Only reduces cravings.
Contraindicated in patients with severe renal disease. Use caution when prescribing for patients with depression and the elderly.
is used to create alcohol aversion
Disulfiram (Antabuse)
MOA Antabuse
a potent alcohol dehydrogenase inhibitor
Safety antabuse
The symptoms can range from flushing, confusion, and vomiting to tachycardia, dyspnea, and syncope. Reactions can even be fatal. The symptoms and intensity are related to the amount of alcohol ingested, the dose of disulfiram, and the time elapsed between medication and alcohol consumption.
Does not eliminate or diminish withdrawal symptoms. This drug is contraindicated for patients with psychosis, severe myocardial disease, or recently used metronidazole. This drug is contraindicated in the concomitant use of alcohol.
