The crew is required and its optional for passengers.

When can you over fly a 100 hr inspection?
You can overfly a 100 hr inspection for the sake of getting it to the mechanic.

An aircraft is involved in an accident which results in substantial damage to the aircraft, the NTSB should be notified?
Immediately.
