Sources of medication information closest to the patient:
Patient
Family member and or other caregivers
Home medication list kept by patient and or their caregiver.
Medication containers-please send bios, adherence packaging) blister or individual packaging./OTC bottles for/package, etc.
PO Box/medication planners
-when obtaining medication history, use at least two source information
-sources close to the patient -
Medication administration record (MAR)
Medication administration records should be available from all healthcare facilities where: - patient reside, 2-medication administer to patient by caregiver
1-ALFs -only if staff responsible for administering medication to resident.
2-LTCFs (assistant living facility)
3-hospice facilities
A note about ALF (assisting living facility):
-most ALF provide resident with an option of managing and administering their medication independently or having ALF staff manage and administer the medication.
-For ALF residence, it is important to find out because it will determine how to proceed.
1-independent with medication’s, move forward with usual data gathering process.
2-if ALF staff administer medication, need a MAR
Medication order VS MARs
-care facility we require a list of medication and other order be signed by a medical provider on a periodic basics example monthly annually.
Medication order are not the same as Mars.
1-it is like that a list of medication order will not reflect resident current regiments.
2-use are the most current sources of medication information, as they are perpetually updated
MARs
-because they are follows by the cure when administering medication Mars should reflect how the patient is actually taking their medication.
Mars include essential information- medication’s, strength/concentration, those, route up administration, frequency:
a-if any medication information questionable or incomplete, facilities, dispensing pharmacy should be contact for clarification.
b-this information should be included on MARs
-may include indication for each medication, but be wary of accuracy
Medication administration records (MAR$) continue
-MARs should be provided by the facility when residents are transferred to the hospital
1-if not, we need to call and ask for them
2-look at the page numbers and put them in order to see if any pages have been admitted.
3-sometime facility have separate medication, administration records, and treatment administration records: be sure to obtain treatment records. Also, as they often include treatment that requires use of medication: eg , with wound care dressing changes required topical medication.
Sources of information from elsewhere:
-electronic medication list from with Ann or outside of the organization example clinic or hospital
Discharge medication list
Pharmacy patient profile/dispense records.
The prescription field data.
Prescription drug monitoring program.
Medication list/medication order from assisted living, long-term care, home, healthcare, and hospice service.
medication administration record (MAR) from assisted-living, long-term care, and hospice facilities
** Important to note that MR are different from paper or electronic medication list because MAO provide medication information and document of administration and perpetually updated as medication changes occur
