HIPAA means:
Health insurance, portability, and accountability act
PHI means
Protected health information
Health Insurance, Portability, and Accountability Act(HIPAA) of 1996:
1-provide the ability to transfer and continue health insurance coverage for American workers and their family when they change or lose the job
2-reduce healthcare, fraud, and abuse
3-mandates industry-wide standards for healthcare information on electronic building and other processes
4-requires the protection and confidential handling of PHI
HIPAA PRIVACY RULE
-applies to healthcare providers who electronically transmit health information
-PHI: individually, identifiable, health information including demographic data, that:
-can identify the individual, or there is reasonable, basic to believe it can be used to identify the individual
-Related to:
1-the individuals pass, present , or future physically, or mentally health or condition
2-the provision or healthcare to the individual
3-the past, present, or future payment to the provision or healthcare to the individual 
HIPAA Privacy rule:
1-apply to healthcare providers who electrically transmit health information.
2-PHI: individually, identifiable, health information, including demographic data,:
3-a cover entity may not use or disclose PHI except as permit or required or if the individual authorize is in writing.
4-a covert entity IS PERMITTED to use and disclose PHI without an individuals authorization for treatment, payment, or healthcare operation
Impact of HIPAA on medication, history-taking:
-/if a clinic or a pharmacy say they cannot share patient on medication list without patient written authorization because of HIPAA: one suggested is to say
ACCORDING TO HIPPA, you are allowed to send me a medication list, without a patient written authorization, because it is forThe PURPOSE OF THE PATIENT TREATMENT’
-politely asked for their supervisor
-you can also get your supervisor involved
-Ideally this issue can be addressed in advance with the area clinic and the pharmacy most frequently utilized by organization patient
Continue impact of HIPAA on medication, history taking
-take step to maintain patient confidentially during conversation with patient and our family caregivers
-Avoid interviewing patient and our family caregiver in hallways and other open spaces
-used to private room when possible
-If in a semi private room, close the curtain and speak relatively soft tone as long as you can be heard by the patient and or family caregiver
-Maintain confidentially of documentation of medication list and other information obtain during medication history taking process
—-look up computer stations
—-do not have written information containing PHI setting in area that I’m not supervisor, and where it may be sent by other who should not have been access to PI
