Baseline
A baseline is the Consumer’s “normal” level of functioning (physical, emotional, mental, and social).
Care plan
Outlines everything the care team is to do to support the Consumer.
Identifies the Consumer’s baseline.
Baseline is what is “Normal” for the Consumer at a certain point in time.
Care team
Provides care for the Consumer.
Includes professionals, relatives, and the Consumer.
Supports the well-being and needs of the Consumer.
Case Manager
Case managers are healthcare professionals who serve as patient advocates to support, guide and coordinate care for patients, families and caregivers as they navigate their health and wellness journeys.
Instrumental Activities of Daily Living (IADLs)
Routine tasks performed around the home or in the community.
Grocery shopping
Housework
Laundry
Meal prep
wood supply
Personal care services
Tasks done to help a Consumer with activities of daily living and instrumental activities of daily living.
Professional boundaries
Clarify your role and job duties often.
Have regular conversations with the Consumer, checking to make sure expectations are understood and being met.
Use the care plan and say “no” to tasks outside of the care plan.
Restraint
A restraint is a device or action that limits or restricts movement of a Consumer from moving or controls Consumers against their will. The use of restraints is illegal.
Self-neglect
Self-neglect occurs when vulnerable adults refuse or are unable to provide goods and services for their:
Physical and mental health
Well-being
Safety
Service Summary
HCA and care team responsibilities.
Contract information for care team members.
Advance directives.
Other people involved in making decisions.
