What is human error?
The failure of planned action to achieve a desired outcome, without the intervention of an unforeseen event
What is verification?
Confirmation that each lifecycle phase has been carried out correctly by progressively checking each phase
What is validation?
Validation assesses the installed SIS to determine if it meets the requirements laid out in the SRS
What is an audit?
An audit is an independent assessment of the procedures honing in on the procedures and methods of implementation of a functional safety systems
What is a functional safety assessment?
A functional safety assessment is an investigation based on evidence to determine if functional safety was achieved
When should an FSA be performed?
At minimum once before the hazard is present. Ideally at each lifecycle stage following the SRS
