Inspect, Palpate - 3
Inspect chest, shape, respiratory movement
Inspect for deformities, abnormal inspiratory retractions, impairment of movement, anterior and posterior
Palpate ribs, trunk for tenderness and deformities
Percuss, auscultate - 2
Anterior lung fields - trachea, above clavicle, pattern down
Posterior and lateral lung fields - along spine, pattern out after scapula, sides especially right side
Chest expansion
Two handed on back, push skin together, see hands come apart
Charting examples - 3
No labored breathing noted
Chest symmetrical with no abnormalities noted on palpation or percussion
Lungs clear to auscultation bilaterally
