Moar cancer stuff PCN and CHL Flashcards
Types of plasma cell neoplasm (PCN)
- MGUS
- Plasmacytoma
- Plasma cell myeloma (Multiple myeloma)
Types of CHL
- Nodular sclerosis
- Mixed cellularity
- Lymphocyte rich
- Lymphocyte depleted
Findings in MGUS
Monoclonal Ig in serum or urine
M protein < myeloma levels
no lytic bone lesions
- NO CRAB symptoms/PCM evidence
MGUS is a precursor to…?
plasma cell myeloma (mult. myeloma)
Pt population of:
- MGUS
- Plasmacytoma
- Plasma cell myeloma (Multiple myeloma)
- MGUS: >50 yr
- Plasmacytoma: 5 yr old, Male (2/3)
- Plasma cell myeloma (Multiple myeloma): >50 yrs
Types of plasma cytoma
- plasmacytoma of bone
2. extraosseous plasmacytoma
plasmacytoma of bone findings
local tumor of bone w/ multiple myeloma like cells
no other lesions
No M protein (like MGUS and PCM)
extraosseous plasmacytoma findings
local mass lesions outside of bone/BM w/ 75% in upper resp. tract
Plasma cell myeloma findings
CRAB:
high Ca2+, renal insufficiency, anemia, bone lesion
M protein in serum or urine (mostly IgG)
Elevated creatinine
Rouleux on smear
Most frequent symptom of Plasma cell myeloma (multiple myeloma)
bone pain (back/extremities),
weakness/tired
CHL driven by?
Hodgkin Reed sternberg cells (RS or HRS),
which are B cell derived
Common markers of CHL
positive for? negative for?
CD15, CD30
Neg: CD19, CD20, CD45
Histo of CHL
multilobular nucleus
eosinophilic inclusions
large cytoplasms
Breakdown % of CHL
- Nodular sclerosis
- 50-80% - Mixed cellularity
- 20-30% - Lymphocyte rich
- 5 % - Lymphocyte depleted
- 1%
Target population of:
- Nodular sclerosis
- Mixed cellularity
- Nodular sclerosis:
- young adults, females - Mixed cellularity:
- Kids/elderly
