Mitral Regurgitation Flashcards

1
Q

What is MR?

A

Leakage of blood backwards through the mitral valve each time LV contracts

Q

What factors increase the annular dilatation and so volume regurgitated through the mitral valve?

A

Annulus enlargement:
Increase in preload, afterload and LV contractility

Annular enlargement -> regurgitant vol. increases

Q

How does the LV compensate for mitral regurgitation?

A

Acute:
End-systolic pressure and volume DECREASE as blood is also pushed through MV to LA as well as aortic v.
Wall tension DECREASES

Chronic:
EDV increases (overload leads to LV to remodel as a thin-walled enlarged chamber)
ESV returns to normal
LVH

Q

How does mitral regurgitation cause atrial fibrillation?

A

The stretching and enlargement of left atrium causes the upper chambers of your heart beat chaotically and rapidly

A

Condition where your heart suddenly can’t pump enough blood to meet your body’s needs

15
Q

Why does the apex beat become hyperdynamic?

A

A valve leak will cause the ventricle to become dilated due to the extra volume of blood within it

16
Q

What will a CXR show in mitral regurgitation?

A

Cardiomegaly
LA enlargement
Calcification of M annulus

17
Q

What will an ECG show in MR?

A
LA enlargement (P > 0.12s, tall)
RVH (prominent R wave in right precordial leads)
18
Q

What will an ECHO show in MR?

A

LV dimensions
Cause of MR:
Leaflets dysfunction
Disease of pap muscles, chord and annulus

19
Q

What can a cardiac MRI be used to assess in mitral regurgitation?

A

Accurate cardiac volumes

Volumetric determination of regurgitant volume

20
Q

What are two surgical treatments for MR?

A

MV apparatus repair

MV replacement
