Basic c section post op care

A
  1. Obs every 5 mins for 30 mins
    1. Temp, pulse, BP, RR
  2. Facial 02 2-5 L
  3. IV access and fluids
  4. Catheter - urinary output
  5. Analgesics
  6. Anti emetics
  7. VTE risk
    1. Stockings, clexane, enoxoparin
  8. ?Antiobiotics
  9. Wound care
  10. Water only 2-4 hours post op –> if water tolerated 1 slice dry toast/biscuit –> 6-8 hours post if all tolerated can start eating
  11. Check for bowel sounds and flatulence passed
  12. 12 hours post op do FBC and mobilise
  13. Discharge for standard c-section 3 days

A

Inflammation of the fetal membranes (amnion and chorion) due to a bacterial infection

Clinical: Fever, tachycardia (mum and baby), uterina tenderness, foul smelling PV discharge

Variability: 5-25

Accelerations (abrupt inc 15 bpm for 15 secs): should occur with uterine contraction

Decels (abrupt dec 15 bpm for 15 secs):

  • Early - start before and finish before contractions normal
  • Variable - at any time and length, are caused by cord compression or oligohydramnios can be resolved be a maternal change in position should be monitored
  • Late - start during and end after contractions sign of uteroplacenta insufficiency abnormal
  • Prolonged - longer than 2 mins quick action e.g. c-section
