A
- Actions that PHNs take on behalf of individuals, families, systems, communities to improve or protect health status
- Defined by type of intervention and level of practice

A
- Population-based
- Intervene at all 3 levels of practice: Individual, community, or systems-focused
- 17 interventions

What are the components of population-based practice?
A
- Focused on entire population
- Grounded in assessment of population’s health status
- Considers broad determinants of health
- Emphasizes all levels of prevention (especially primary)
- Intervenes with communities, systems, individuals, and families

What is the difference btw population of interest and population at risk?
A
- Population of interest: group of people you want to know more about (ex. senior citizens)
- Population at risk: group of people exposed to or with a certain factor (ex. senior citizens with HTN)

What are the 3 levels of PHN practice?
A
- Individual-focused: changes in knowledge, attitudes, beliefs, practices, and behaviors of individuals
- Community-focused: changes in community norms, attitudes, awareness, practices, behavior
- Systems-focused: changes in organizations, policies, laws, power structures (often more effective and long-lasting way to impact population health)

Exceptions to 3 levels of practice
A
- Case finding: only occurs at individual level
- Community organizing and coalition building: cannot occur at the individual level, only at community/system level

Structure of Minnesota Wheel
A
- Grouped together by similarities
- Right side: primarily individual, families, groups
- Left side: primarily systems and communities
- Usually work in more than one wedge at a time

- Surveillance: describe and monitor health events
- Investigation: gather and analyze data regarding threats to the health of pop
- Outreach: locates pop of interest or risk
- Screening: identify individuals with risk factors or disease conditions
- CASE FINDING OCCURS at individual level of these

- Referral and follow up: assist people to identify and access necessary resources
- Case management: optimize self-care capabilities of individuals and families
- Delegated functions: direct care tasks carried out by RN

- Health teaching: communicates facts, ideas, skills
- Counseling: establishes interpersonal relationship at the emotional level
- Consultation: seeks information and generates optimal solutions to perceived problems

- Collaboration: commits 2+ persons/organizations to achieve a common goal
- Coalition building: develops alliances among organizations for a common purpose
- Community organizing: helps community groups to identify common problems and meet goals

- Advocacy: pleads someone’s cause or act on someone’s behalf, with a focus on developing the capacity or community/system/individual/family
- Social marketing: utilizes commercial marketing principles to influence knowledge and practices
- Policy development and enforcement: places health issues on agendas, results in laws and compels others to comply with them
