Q

What are the features of Minimal Change Disease?

A

nephrotic syndrome

normotension - hypertension is rare

highly selective proteinuria

only intermediate-sized proteins such as albumin and transferrin leak through the glomerulus

renal biopsy-

normal glomeruli on light microscopy

electron microscopy shows fusion of podocytes and effacement of foot processes

Q

Why is there low albumin level in Minimal Change Disease?

A

only intermediate-sized proteins such as albumin and transferrin leak through the glomerulus - hence there’s proteinuria and is classified as a nephrotic syndrome.

Q

What features would make you differentiate nephrotic and nephritic syndrome in relation to MCD?

A

MCD IS pure nephrotic syndrome with normotension.

IF the dipstick is positive for proteins BUT it is also highly positive for blood, IT INDICATES a nephritic syndrome.
