Inhibit transpeptidase (cell wall synthesis). Penicillins, cephalosporins, aztreonam, imipenem
Bactericidal
Beta lactams, fluoroquinolones, aminoglycosides, metronidazole
Act on cell wall
Beta lactams, vanco, bacitracin, polymyxins
Act on 30S
Aminoglycosides, tetracyclines
Act on 50S
Linezolid, clindamycin, chloramphenicol, macrolides
Penicillin
Beta lactam. Syphilis. SE: hypersensitivity, rash, hemolytic anemia.
Aminopenicillins
Ampicillin, amoxicillin.
Enterococcus, listeria. 2nd line for Lyme.
SE: hypersensitivity, rash, pseudomembranous colitis
Piperacillin
Pseudomonas
Cephalosporins
Beta lactam.
1st gen-cefazolin, cephalexin
2nd gen-cefotetan, cefuroxime
3rd gen- cefoperazone, ceftazidime, ceftriaxone
SE: 5-10% pts with penicillin allergy also react to cephs. Ohm dome have disulfiram like reaction.
Aztreonam
Beta lactam. G- rods. Safe in pregnancy. Usually nontoxic.
Carbapenems
Imipenem/cilastatin, meropenem. Resistant G- infections. Cilastatin prevents renal metab of imipenem.
SE: hypersensitivity and rash, GI distress, drug fever, seizures if renal dysfunction (less with meropenem)
