Estrogens and Progestins
List the estrogens of MHT:
Estradiol, Conjugated Estrogens, Esterfied Estrogens, Estropipate (generic)
What do estrogens dec? Inc?
Dec activity of: cholesterol, anti-thrombin III, osteoclasts
Inc activity of: clotting factors, platelet aggregation, Na/fluid retention, thyroid binding globule
Risks w/ estrogens?
Endometrial hyperplasia, DVT/PE/stroke, breast CA
List the Progestins:
Medroxyprogesterone (MPA), Methyltestosterone, Progesterone
What is the purpose of progestins?
To oppose estrogen’s neg side-effects (uterine CA)
When are Progestins not indicated?
In women who have no uterus
What is the SERM? What’s its indication? Risks?
Ospemifene Dyspareunia (painful intercourse) Worsening hot flashes/sweating, inc stroke risk, endometrial thickening
What is Ospemifene’s MOA?
Selective estrogen R compound, estrogen agonist in vagina, anti-estrogenic in breast
Inc cell growth, secretions, dec pH, dec pain
What are the estrogen Rs?
Alpha and beta
What is the TSEC?
Bazedoxifene
What is Bazedoxifene’s indication?
Women w/ intact uterus, moderate-to-severe vasomotor sxs d/t menopause
Risks involved w/ TSECs?
It’s Bazedoxifene..
Stroke, worsening hot flashes
TSEC MOA?
It’s Bazedoxifene..
Ant in endometrium and breast, agonist in bone
