Stress management
Sleep: treat sleep apnea if confirmed
Diet: Mediterranean, DASH (sodium limited to 2300 mg/day), low glycemic index, high fibre (30+ g/day)
Weight loss: aim for 5-10% loss of initial weight, ultimate goal BMI <25
