What are the 3 clusters (broad strokes) of personality disorders?
A - Odd/eccentric: Schizoid, Schizotypal, Paranoid
B – Dramatic/Emotional: Borderline (BPD), Narcissistic, Antisocial, Histrionic
C – Anxious/Fearful: Dependant, Avoidant, Obsessive-Compulsive Personality Disorder (this is NOT The same at OCD)

What is a personality disorder?
Enduring pattern of inner experience & behaviour that deviates markedly from expectations of one’s culture.
Manifests in 2+
- cognition - affectivity - interpersonal functioning - impulse control

distrust and suspiciousness of others such that their motives are interpreted as malevolent

detachment from social relationships and a restricted range of expression of emotions in an interpersonal setting

social and interpersonal deficits marked by acute discomfort with, and reduced capacity for, close relationships as well as by cognitive or perceptual distortions and eccentric behaviour

Borderline PD factors associated w/ remission
Remission occurs in 45%
- 35% within 2 years
- 91% within 10 years
- 78% of these had sustained remission for > 8 years
Factors that are associated w/ shorter time to remission:
- Age > 25
- Good functioning at work or school in 2 years prior
- No hx childhood sexual abuse
- No hx substance misuse
- Absence of comorbid anxiety disorder
- Low neuroticism
- High agreeableness and affiliating tendencies
Recurrence is common!

Signs and symptoms of borderline personality disorder (3 main categories)
- impaired relatedness (identity disturbance, unstable relationships)
- affective dysregulation (anger, affective labillity)
- behaviour dysregulation (impulsivity, suicidality, self-injury)

Borderline PD red flags
8-12% die by suicide
Take ALL attempts/ideation/plans/threats seriously
Factors for chronic risk:
- impulsivity, negative affectivity, poor psychosocial fntn
Factors for acute risk:
- recent depression
- adverse life events
- substance misuse
- recent loss

Borderline PD Dx Criteria
5 or more:
1. Frantic efforts to avoid real or imagined abandonment.
- A pattern of unstable and intense interpersonal relationships characterized by alternating between extremes of idealization and devaluation.
- Identity disturbance: markedly and persistently unstable self-image or sense of self.
- Impulsivity in at least two areas that are potentially self-damaging
- Recurrent suicidal behavior, gestures, or threats, or self-mutilating behavior.
- Affective instability due to a marked reactivity of mood
- Chronic feelings of emptiness.
- Inappropriate, intense anger or difficulty controlling anger
- Transient, stress-related paranoid ideation or severe dissociative symptoms.
