What is it ?
Lack of blood flow through the mesenteric vessels supplying the intestines resulting in ischaemia
What is chronic mesenteric ischaemia
Narrowing of the mesenteric blood vessels by atherosclerosis
What is the presentation of chronic mesenteric ischaemia?
- Central colicky abdo pain after eating
- Weight loss - avoids food due to pain
- Abdo bruit
how is chronic mesenteric ischaemia diagnosed?
CT angiography
What is the management of chronic MI?
Reduce modifiable risk factors
secondary prevention
Revascularisation
How is revascularisation performed?
Endovascular procedures - percutaneous mesenteric artery stenting
Open surgery e.g. endarterectomy, bypass
What is a key risk factor of acute MI?
AF - thrombus forms in left atrium then mobilises down aorta to superior mesenteric artery
How does acute mI present?
Acute
Non-specific abdominal pain
Pain disproportionate to exam findings
What is the test for acute mesenteric ischaemia?
Contrast CT
What results may you see on VBG in acute mi?
metabolic acid
raised lactate
What is treatment for acute mesenteric ischaemia ?
Remove necrotic bowel
Remove or bypass thrombus
