Rx requirements for vet Rx
- name address, tel, qualification and signature of prescriber. FOR CD: RCVS REG NUMBER
- name and address of owner
- identification and species of animal
- date. normal Rx valid for 6 mths. CD RX VALID FOR 28 DAYS.
- name, quantity, dose and admin instructions - AS DIRECTED NOT OKAY
- warnings and withdrawal period if necessary
- If human med is prescribed and not licensed for use in animals: “PRESCRIBED UNDER THE VETERINARY CASCADE”
- FOR CD2,3: THE ITEM HAS BEEN PRESCRIBED FOR AN ANIMAL OR HERD UNDER THE CARE OF THE VETERINARIAN
- if Rx is repeatable (not a CD) the number of repeats required.
VETERINARY CD RX NEED TO BE RETAINED FOR 5 YEARS AND NOT SENT TO NHS AGENCY.
