germline mutation in melanoma
Rarely, melanoma can arise in the context of a germline mutation in the oncogene BAP1, which is associated with the development of cutaneous and uveal melanomas, as well as other cancers including renal cell carcinoma and mesothelioma.
work-up based on risk melanoma
SLN + then get CT (essentially stage 3 disease)
Complete node dissection in melanoma
Not needed. Can do surveillance instead.
if nodal basin recurrence only in melanoma then…
remove all lymph nodes in the basin
BRAF inhibitor side effects
photosensitivity, arthralgias, SCC, GI side effects and fevers with MEK
when to do radiation in melanoma
extra capsular extension
Merkel cell carcinoma
rows rapidly and may be small, but 30% of cases have lymph nodes involved; therefore SLNB is recommended in all cases for staging.
