- CHF
- Alcohol abuse
- Metabolic acidosis
- Liver or kidney disease
- Pt is >80 yrs old

- Kidney surveillance testing required
- Monitor serum creatinine - Take w/ food
- Discontinue after OR, dye (contrast) studies for 48 hrs or until kidney function normal
