Morphine role in MI
Relieve chest pain
Reduce anxiety
Decrease O2 demand of the heart & GI motility
Caution clients who have asthma/emphysema –> resp. depression
Aspirin action, purpose, nurse actions in coronary artery disease
Action: Decrease clot formation
Purpose: prevent total blockage of coronary artery
Nursing action: Use cautiously in at risk for bleeding pts.
Nitroglycerin action, nursing actions, side effects
-Action: Prevents coronary artery vasospasm & reduces pre/afterload = decreasing O2 demand
Vasodilation + lowers BP
TX: angina
-Nursing action: Pt needs to be sitting or laying down
Administer 0.4 sublingual DO NOT SWALLOW (assess BP & pain baseline)
If pain is unrelieved in 5 min call 911, or be driven to ED
Side effects: headache is common, ortho hypotension
Beta blockers (action, nursing actions, side effects)
Action: Antihypertension, lowers BP, antidysrhythmic, lowers HR
Nursing action: Assess BP, hold if less than 60 bpm
Monitor: LOC, crackles, chest discomfort
Side effects: Bradycardia, hypotension
Atropine
-For symptomatic bradycardia
-Raises BP
Chest pain, SOB, fatigue, cool/clammy skin, syncope
Administer 0.5 increments up to 3 mg only!!!
Adenosine
For SVT: causes sinus pause then sinus rhythm
Nursing action: brachial IV access
Need to push NS bolus within 3 seconds to help med reach the heart
First dose: 6mg
Second dose: 12 mg
Digoxin
Action: helps w/ cardiac rhythm like arrhythmias (AFIB)
Toxicity: headache, irritation, not eating, photosensitivity
Nursing action: assess apical pulse for 1 minute, hold if <60 bpm
Assess, resp rate & POTASSIUM LEVEL!! 3.5-5.0
